Physical Therapist – Home Health Travel Contract in Vancleave, MS
Travel Physical Therapist opportunity in Vancleave, Mississippi: support a home health caseload, help patients regain independence, and deliver high-quality rehab care across Jackson County. - Estimated Weekly Pay: $2,030 – $2,190
- Weekly Hours: 40 guaranteed hours
- Shift Duration: 8-hour shifts
- Schedule: Monday-Friday with on-call and weekend rotation
- Coverage Area: Jackson County, MS
- Productivity Expectations: 32 points
- Start Date: 08/10/2026
Job Requirements
- Active Physical Therapist license in the state of Mississippi
- Minimum of 1 year of Physical Therapy experience preferred
- Home health experience required
- HCHB experience required
- OASIS experience required
- Master’s degree required
- Strong professionalism, flexibility, and teamwork skills
- Ability to work independently in a home health setting
Responsibilities
- Perform patient assessments, evaluations, and treatment plans
- Complete OASIS documentation accurately and in a timely manner
- Supervise PTA services as needed
- Develop and update individualized plans of care
- Submit documentation by 8:00 AM the next morning
- Provide high-quality rehabilitation care across the assigned home health coverage area
- Coordinate effectively with the care team to support positive patient outcomes
