Home Health Physical Therapist Job Details
- Location: New Braunfels, TX
- Healthcare Setting: Home Health
- Position: Travel Physical Therapist (PT)
- Weekly Estimated Pay: $2,030-$2,190
- Contract: 13 weeks
- Start Date: October 12, 2026
- End Date: January 10, 2027
- Schedule: Approximately 40 hours per week; 8-hour shifts, typically 8:00 a.m.-4:30 p.m.
- Weekly Hour Guarantee: 36 hours
- Coverage Area: Approximately a 60-mile radius around New Braunfels
- Call/Weekend Requirement: No regular on-call or weekend requirement
- Travel: Mileage between patient visits is reimbursed
Physical Therapist Job Requirements
- Bachelor’s degree in Physical Therapy or a related field
- Valid authorization to practice as a Physical Therapist in Texas
- Professional certification or licensure required for practice
- At least 1 year of experience as a Physical Therapist
- Strong patient assessment, clinical documentation, communication, and care-planning skills
- Home health Physical Therapy experience preferred
- Experience with Homecare Homebase (HCHB) is a plus
- Ability to travel throughout the assigned service area and provide skilled care in patients’ homes
Home Health Physical Therapist Responsibilities
- Provide skilled Physical Therapy services to patients in their homes
- Evaluate patients’ mobility, strength, balance, functional abilities, and rehabilitation needs
- Develop and implement individualized Physical Therapy treatment plans aligned with patient goals
- Perform therapeutic exercises, functional training, mobility interventions, and patient education
- Monitor patient progress and modify treatment plans as clinically appropriate
- Educate patients and caregivers on home exercise programs, safety techniques, and strategies that support independence
- Document assessments, interventions, progress, and care coordination activities accurately and promptly
- Collaborate with patients, caregivers, nurses, physicians, and other members of the healthcare team
