Home Health LPN Job Details
- Weekly Estimated Pay: $1,370-$1,440
- Location: Virginia Beach, Virginia
- Healthcare Setting: Home Health
- Assignment Dates: October 5, 2026-January 8, 2027
- Contract Duration: Approximately 14 weeks
- Schedule: Monday-Friday, 8:30 a.m.-5:00 p.m.
- Shift Duration: 8-hour shifts
- Hours: 40 guaranteed hours per week
- On-Call/Weekend Requirement: None
- Travel: Mileage between patient visits is reimbursed
Licensed Practical Nurse Job Requirements
- Active LPN license in the state of practice
- At least 1 year of professional nursing experience, including home health experience
- High school diploma, GED, or equivalent
- Strong clinical judgment, organization, and communication skills
- Ability to work independently while maintaining accurate clinical documentation and timely communication with the care team
- Reliable transportation and the ability to travel between patient visits
- Homecare Homebase experience is preferred
- Willingness to wear scrubs in any color
Home Health LPN Responsibilities
- Provide routine skilled nursing visits to patients in their homes
- Assess patient needs and deliver home health care in accordance with individualized care plans
- Perform wound care, including management of wound vacs
- Collect laboratory specimens and complete ordered clinical procedures
- Provide catheter and Foley care
- Educate patients and caregivers on medications, treatment plans, disease management, and home safety
- Assist with medication education and management
- Monitor patient progress, identify changes in condition, and communicate concerns promptly
- Maintain complete and accurate clinical documentation for each patient visit
