• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Shift Duration: 8-hour day shifts
• Shifts Per Week: 5
• Contract Length: 13 weeks
• Estimated Weekly Pay: $2,280 – $2,450
• Patient Volume: Approximately 5-6 patients per day
• Coverage Area: Kent County with travel up to 75 miles from the assigned home base
• Documentation System: HomeCare HomeBase (HCHB)
• Productivity Expectation: 30 points per week
• Hours: No guaranteed hours

Job Requirements

• Minimum of 2 years of Physical Therapy experience preferred
• At least 1 full year of adult home health experience required
• Recent home health experience required
• Current Physical Therapist license required and must be in hand
• CPR/BLS certification required
• Proficiency with HomeCare HomeBase EMR required
• Reliable transportation, valid driver’s license, and auto insurance required
• Comfortable driving to patient homes daily and traveling between visits within a 50-75 mile radius
• Strong skills in assessments, evaluations, plan of care, discharge planning, reassessments, and time management

Responsibilities

• Provide skilled physical therapy visits in the home health setting
• Complete evaluations, follow-up visits, reassessments, and discharge planning
• Develop and update individualized plans of care to support mobility, safety, and functional independence
• Deliver care for start-of-care and follow-up visits with attention to patient goals and outcomes
• Document all care accurately and within 24 hours of each visit
• Maintain weekly productivity expectations while managing a full caseload
• Travel to patient homes and attend periodic office meetings as required
• Work collaboratively with the care team to support positive patient progress and quality outcomes
