Job Requirements

• BSN required; master’s degree in nursing, health administration, or business administration preferred
• Active RN license and experience in an acute-care healthcare setting required
• Minimum 5 years of progressively responsible leadership or administrative experience in case management, care coordination, utilization management, or a closely related function required
• Case management certification such as CCM or ACM preferred
• Strong knowledge of case management, discharge planning, patient progression, and transitions of care in an acute-care environment
• Demonstrated experience supporting interdisciplinary clinical and operational programs, staff leadership, performance improvement, and regulatory or accreditation readiness

Responsibilities

• Lead and support patient care coordination efforts to promote safe, efficient transitions of care
• Collaborate with interdisciplinary healthcare teams to support discharge planning and utilization management
• Oversee patient progression and help ensure timely movement through the continuum of care
• Support staff development, operational excellence, and performance improvement initiatives
• Contribute to regulatory and accreditation readiness efforts across the care coordination function
• Use advanced case management knowledge to help optimize patient outcomes and resource utilization
