Physical Therapist Job Details

• Weekly Estimated Pay: $2,310-$2,480
• Assignment Length: 13 weeks
• Start Date: October 19, 2026
• Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m.
• Shifts per Week: 5
• Shift Duration: 8-hour days
• Hours: 40 hours per week
• Work Setting: Home health
• Caseload: Approximately 6-7 weighted visits per day
• Weekend Coverage: Approximately every fourth or fifth weekend for Physical Therapy; weekend coverage may also be needed for orthopedic cases

Home Health Physical Therapist Requirements

• At least 1 year of Physical Therapist experience required
• Home health physical therapy experience preferred
• Active Virginia Physical Therapist license or compact privilege required at submission
• CPR certification required
• Experience with MatrixCare or Epic preferred, but not required
• Strong clinical judgment, communication, organization, and clinical documentation skills
• Ability to provide safe, independent patient care in home-based environments

Physical Therapist Responsibilities

• Evaluate patients and develop individualized, medically prescribed physical therapy treatment plans
• Provide therapeutic interventions to restore function, relieve pain, and prevent disability
• Educate patients and caregivers on treatment plans, home exercise programs, mobility, and safety
• Direct patients through selected activities designed to restore, reinforce, and enhance physical performance
• Monitor patient progress and modify treatment plans as clinically appropriate
• Participate in discharge planning and coordinate care with the interdisciplinary team
• Complete accurate and timely clinical documentation in accordance with applicable policies and standards
