Home Health Registered Nurse (RN) – Travel Contract in South Dennis, MA

Job Details
- Weekly Estimated Pay: $2,250 – $2,370
- Setting: Home Health / Visiting Nurse Association

- Schedule: Monday-Friday with occasional weekends and holidays
- Float Coverage: Upper Cape area, with occasional floating to Mid Cape
- Employment Type: Travel RN Contract

RN Job Requirements
- Active Massachusetts RN license
- Minimum 1 year of nursing experience required, or 6 months of nursing experience with a BSN
- Current CPR certification required
- Successful completion of pre-hire medication test
- Home health nursing and IV experience preferred
- Strong computer and clinical documentation skills preferred
- BSN preferred
Home Health RN Responsibilities
- Complete comprehensive nursing assessments at admission
- Develop, implement, and revise individualized plans of care
- Use clinical judgment to determine visit frequency and coordinate interdisciplinary care
- Monitor patient condition and notify providers of significant changes
- Participate in interdisciplinary case conferences and evaluate plan-of-care effectiveness
- Provide and document supervision of Home Health Aide care every 14 days
- Complete admissions, skilled visit documentation, and discharge paperwork accurately and on time
- Obtain and clarify valid provider orders for all care delivered
- Educate patients and caregivers while documenting skilled nursing care clearly and thoroughly
- Coordinate safe discharges and connect patients with community resources as appropriate
