Licensed Practical Nurse (LPN) – Long-Term Care in Penn Yan, NY

- Weekly Hours: 40 hours per week
- Shift Duration: 8-hour shifts
- Shifts Per Week: 5 shifts per week
- Shift Schedule: Day and evening shifts as needed
- Weekend/Holiday Requirement: Every other weekend and holidays required
- Estimated Weekly Pay: $1,560 – $1,640
Job Requirements
- Active New York LPN license
- Current BLS certification through AHA or RQI
- At least 1 year of LPN experience required
- High School Diploma or GED required
- Experience in long-term care, skilled nursing, or a similar adult/geriatric setting preferred
- Ability to start on a Tuesday or Thursday for full-shift orientation
- Comfortable providing care for adult and geriatric residents
Responsibilities
- Provide direct and indirect resident care within the LPN scope of practice
- Support activities of daily living, including hygiene, grooming, dressing, toileting, incontinence care, turning, positioning, and skin care
- Assist with resident safety measures, including call lights, alarms, low beds, safety mats, and fall prevention
- Help meet nutritional needs through meal support, tray preparation, feeding assistance, and honoring resident preferences
- Support restorative care such as range of motion, ambulation, transfers, body alignment, and assistive device use
- Document care accurately and promptly according to facility protocols
- Report resident concerns, refusals, and changes in condition to the Charge Nurse or Supervising Nurse in a timely manner
- Maintain a safe, clean, and comfortable environment for residents
