Job Details

Job Requirements

• Degree in Physical Therapy from an accredited school
• Active Physical Therapist license for the state where the assignment is performed
• At least 1 year of outpatient clinical experience
• Current CPR certification
• Strong communication, documentation, and clinical reasoning skills
• Ability to work independently while collaborating with an interdisciplinary healthcare team

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical findings and physician direction
• Implement and modify outpatient physical therapy interventions to support patient progress toward discharge goals
• Document evaluations, treatment plans, progress notes, goal updates, and related records accurately and on time
• Provide patient, family, and team education to support recovery, mobility, and functional outcomes
• Coordinate with providers and care teams to ensure treatment aligns with approved orders and clinical needs
• Support a safe care environment by following established safety practices and clinic guidelines
• Contribute to quality improvement efforts and ongoing professional development
• Assist with student learning experiences when assigned and appropriate
